Autism and GI conditions

Gastroesophageal Reflux Disease (GERD)
Gastroesophageal Reflux Disease (GERD) occurs when stomach acid refluxes into the esophagus. This is one of the better-understood GI conditions in autism. Mostafavi outlines a recent study that found that people with autism have a higher risk of developing erosive esophagitis and esophageal ulcers compared to non-autistic groups (10:20). She remarks that acid-blocking medications can mitigate some risk and describes the BRAVO wireless and nasal probe tests for GERD (10:20).

Inflammatory bowel disease
Inflammatory bowel disease (IBD) is broadly characterized by Crohn’s disease and ulcerative colitis, which are caused by inflammation of the GI tract. A recent systematic review and meta-analysis (6 studies, 11 million participants) found an association between autism and the later development of IBD. Mostafavi notes that children with autism often have more potent treatments (second-tier biologics) for GI conditions compared to non-autistic children with GI issues, underscoring their severity in autism (14:10).

Eosinophilic GI disease (EGID)
Eosinophilic GI disease (EGID) looks at the entire GI tract and is associated with an imbalance in the immune system potentially related to food sensitivities. Over time, complications can include fibrosis and eosinophilic esophagitis (EOE). The speaker outlines a recent systematic review and meta-analysis (6 studies, +700,000 participants), which revealed an association between autism and EGID, where the prevalence of autism in the EGID population is 21.59% (18:00).
Avoidant/restrictive food intake disorder (ARFID)
Avoidant/restrictive food intake disorder (ARFID) is related to at least one of the following:
- Fear of consequences (symptoms that the food triggers)
- Sensory aversion
- Lack of interest/awareness of appetite cues
Unlike other eating disorders, ARFID is not related to body appearance. Mostafavi notes inadequate caloric intake and deficiencies in micronutrients and vitamins as concerns associated with ARFID. Recent research shows a significant relationship between autism and ARFID, and sensory issues are the most commonly described driver (22:10). The presenter suggests trying treatments like oral desensitization and pairing foods and asserts that applying what works in other intervention spheres to ARFID may be possible (25:20).

Disorders of the gut brain interaction (DGBI)
The gut-brain access involves both nervous systems; many neurotransmitters are produced in our guts. Mostagavi asserts that just because labs come back negative does not mean that GI symptoms are not real because there is no specific test for DGBI (27:15). A retrospective study found that nearly one-third of participants seen through an autism-specific clinic experienced functional GI conditions. The speaker underscores how difficult pain localization can be for people with autism due to complications with interoception (31:00).

Autism and constipation
Constipation is one of the most reported GI symptoms associated with autism. However, chronic constipation does not seem to be associated with a higher rate of abnormal colonic motility in autism. Mostafavi outlines research showing that children with autism are more likely to visit the ER and be admitted to the hospital for constipation-related issues compared to children with other or no chronic conditions. This significantly increases healthcare costs and utilization, increasing the chances of poor quality or lack of treatment (33:00).

Resources and tips
The presenter notes a 2010 article published in Pediatrics as a good reference for GI issues in autism. The article talks about constipation, guidelines for evaluation in patients with high-risk, mitochondrial conditions, and medication (37:00). Objective:
To perform a large registry-based study to determine the relative prevalence of gastrointestinal (GI) problems in children with an autism spectrum disorder (ASD) from families with multiple affected members compared with their unaffected sibling(s).
Methods:
In-home structured retrospective medical history interviews by parent recall were conducted by a pediatric neurologist. Our analysis sample included information about GI health of 589 subjects with idiopathic, familial ASD and 163 of their unaffected sibling controls registered with Autism Genetic Resource Exchange. Individuals with ASD were subgrouped into 3 autism severity groups (Full Autism, Almost Autism, and Spectrum) based on their Autism Diagnostic Interview—Revised and Autism Diagnostic Observation Scale scores.
To conduct a meta-analysis of research investigating GI symptoms among children with ASD.
We searched Medline, PsycINFO, and PubMed databases (1980–2012) in peer-reviewed journals. Analysis involved studies with a comparison group presenting quantitative data on GI symptoms using combinations of terms for ASD and GI indicators. The systematic search yielded 15 studies. We calculated effect sizes and 95% confidence intervals (CIs) using a random-effects model.
Autism spectrum disorder (ASD) is a multifactorial, pervasive neurodevelopmental disorder affecting 1 in 36 children in the United States. Given the rising prevalence and significant economic and social costs associated with ASD, it is critical that continued efforts be made towards better understanding the underlying etiology as well as management of the condition and its commonly associated comorbidities. It has been estimated that upwards of 70% of children with ASD have a positive history of gastrointestinal (GI) symptoms. In this retrospective, descriptive study, we identified 131 patients with diagnosis of autism spectrum disorder who presented for initial evaluation by pediatric gastroenterology at the Baystate Children’s Specialty Center. We collected data from chart review of these patients with a particular focus on reason for referral, components of evaluation as well as results of said evaluation. Of the 131 patients, the most frequent reason for referral included constipation (42.7%), abdominal pain (27.5%), and feeding difficulties (26.7%). After completion of the evaluation, 60.3% of patients were ultimately diagnosed with a functional gastrointestinal condition. Of patients who completed endoscopic evaluation, 40% of patients were found to have grossly abnormal and 40% were found to have pathologically abnormal EGD. The majority of patients were recommended to have diagnostic evaluation; however, a large proportion of them were unable to complete said evaluation. The majority of patients were found to have abnormal testing; however, the majority of patients were additionally diagnosed with a functional gastrointestinal condition.
Objectives: Children on the autism spectrum disorder (ASD) may express pain or discomfort through stereotypic or self-injurious behaviors. Gastroesophageal reflux disease (GERD) may be challenging to diagnose in a child who is non-verbal or has impaired communication skills, diagnostic testing for GERD may be the only way to establish the diagnosis. We report our experience using the BRAVO wireless pH monitoring device for the evaluation of GERD in this patient population.
Methods: Tolerance and feasibility as well as pH parameters and symptom correlation of the BRAVO pH were evaluated retrospectively in ASD children and compared it to a large cohort of non-ASD children. Only patients with studies lasting >24 hours were included.
Autism spectrum disorders (ASD) and inflammatory bowel disease (IBD) both have multifactorial pathogenesis with an increasing number of studies demonstrating gut-brain associations. We aim to examine the association between ASD and IBD using strict classification criteria for IBD. We conducted a retrospective case-cohort study using records from the Military Health System database with IBD defined as having one encounter with an ICD-9-CM diagnostic code for IBD and at least one outpatient prescription dispensed for a medication to treat IBD. Children with ASD were more likely to meet criteria for Crohn’s disease (CD) and Ulcerative colitis (UC) compared to controls. This higher prevalence of CD and UC in children with ASD compared to controls confirms the association of ASD with IBD.
Introduction: Autism spectrum disorder (ASD) is a heterogeneous group of neurodevelopmental disorders with complex multifactorial etiologies. Medical comorbidities are common in ASD and include functional gastrointestinal disorders (fGID), which are reported in 30–70% of patients. In this research study, we aimed to systematically assess the prevalence of gastrointestinal problems in ASD and describe their clinical correlates.
Methods: In this retrospective study, we reviewed the medical records of all patients admitted to the Comprehensive Medical Program for ASD (AMITEA) at Gregorio Marañón University General Hospital from January 2012 to December 2015. All patients fulfilled the clinical criteria for ASD (DSM-IV-TR). In addition to fGID, epidemiological and clinical variables were collected at intake. The primary objective was to determine whether chronic constipation is associated with a higher rate of abnormal colonic motor activity in ASD children than in non-ASD children. A secondary goal was to determine if clinical variables could identify children with ASD at risk for possessing abnormal colonic motility.
Methods: A retrospective, propensity-matched, case-control study compared colonic manometry (CM) of an ASD cohort and non-ASD controls with chronic constipation. Clinical variables were evaluated as potential predictors for abnormal colonic motility.
